Community Dental Care, a nonprofit dental clinic with five locations throughout Minnesota, recently suffered a data breach that involved patient information.

The nonprofit said it launched an investigation after becoming aware of unauthorized activity in its computer system in December. That investigation determined that certain personal information was accessed or acquired without authorization by an unknown actor on or about Dec. 6. The clinic completed a review of the impacted data March 24.

The potentially impacted information may have included names, addresses, Social Security numbers, medical information and health insurance information, among other data, according to an April 1 news release.

The clinic said it reported the incident to the FBI, the HHS Department’s Office for Civil Rights and other consumer reporting agencies. It also took steps to reduce the risk of similar incidents happening in the future.
