Fairfax Oral and Maxillofacial Surgery, which has six locations in Virginia, experienced a data security incident where certain files on the practice's system were encrypted.
The unauthorized party that encrypted the files accessed the systems between May 15 and May 16, according to a post on Fairfax Oral and Maxillofacial Surgery's website.
On July 11, the practice's investigation determined that the encrypted files included personal information including name, driver's license number, Social Security number, health insurance information and medical information.
The practice is not aware of any instances where any personal data has been misused for committing fraud or identity theft, the post said.
