North Dakota: 59 percent
Montana: 56.9 percent
North Carolina: 43.7 percent
Iowa: 40.4 percent
Massachusetts*: 38.7 percent
California: 34.3 percent
Connecticut: 27.3 percent
Wisconsin: 27.1 percent
Washington: 25.8 percent
Rhode Island: 25.5 percent
*States that enroll only certain segments of Medicaid enrollees in managed care programs or provide only certain services through managed care programs.
